About FLPPS

In 2014, New York State, in conjunction with the federal government, began a major initiative – the Delivery System Reform Incentive Payment (DSRIP) Program. DSRIP’s purpose is to fundamentally restructure the health care delivery system by reinvesting in the Medicaid program, with the primary goal of reducing avoidable hospital use by 25% over 5 years.

Across New York State, numerous groups participate in accomplishing DSRIP. The Finger Lakes Performing Provider System (FLPPS) is a partnership comprised of 19 hospitals, 6,700 healthcare providers, and more than 600 healthcare and community-based organizations in a 13-county region (Allegany, Cayuga, Chemung, Genesee, Livingston, Monroe, Ontario, Orleans, Seneca, Steuben, Wayne, Wyoming and Yates counties). FLPPS Partners collectively serve over 300,000 Medicaid beneficiaries and 100,000 uninsured individuals.

THE CHALLENGE:

Help Partners transition to a new healthcare model envisioned and promoted by DSRIP

FLPPS wanted a set of elearning courses to help its partners understand and accomplish the changes necessary to transition from a healthcare model that compensates based on the volume of services to the value-based healthcare model envisioned and promoted by DSRIP.

Part of what makes this transition challenging is the broad range of organizations that are FLPPS partners. The individuals within these many organizations vary greatly in their medical, operational, and financial focus and background.

It personalizes some of the perspectives of different patients, so that people start to understand these topics on a deeper, more individual level, and it does so in a way that is engaging, interactive and based on adult learning principles.”

OUR SOLUTION:

Create two suites of eLearning courses to accommodate the foundation and home health care segments

Given this landscape, FLPPS envisioned a series of foundational modules, for everyone involved.

In addition, there would also be more specialized learning tracks with modules for specific roles, such as home health aides, community health workers, and behavioral health workers, to focus on their particular tasks to achieve the successful transition to value-based healthcare.

Illumina and FLPPS collaborated to create the foundation series and the home health aide series.

RESULTS:

The Foundation Series starts with an elearning module that provides an introduction to value-based healthcare. FLPPS summarizes how the 15-20 minute course functions to achieve its objectives:

It successfully allows the learner to digest the topic on a broad level before drilling down into more specific topics in future modules/learning tracks.

It also gives an overview of a topic that traditionally has been very hard to break down and understand. It personalizes some of the perspectives of different patients, so that people start to understand these topics on a deeper, more individual level, and it does so in a way that is engaging, interactive and based on adult learning principles.
